CARE in Practice: Moving from CARE Planning to Graduation Planning

This CARE in Practice will provide an overview of CARE process and components moving from CARE planning to graduation planning, followed by a panel discussion to learn about how other counties are integrating innovative practices around graduation planning.

Learning Objectives

At the end of the session, participants will have an increased ability to:

  • Explore strategies for moving from CARE planning to graduation planning, including outreach, ongoing engagement, court participation, care planning, and person-centered care.
  • Gain insights from other counties on successful models, adaptations, and lessons learned in organizing around graduation.
