Paths Out of CARE
In July, the Paths Out of CARE training and panel session focused on supporting successful transitions out of the CARE process. The session emphasized legal pathways, person-centered planning, and best practices for continuity of care following CARE.
Main Transition Scenarios
The CARE Act outlines several structured pathways through which individuals may exit the program, depending on their engagement and progress. These include:
- Voluntary engagement leading to early dismissal.
- Completion of a CARE agreement.
- Graduation from a CARE plan.
Other Exit Paths
In addition to the primary scenarios, there are alternative routes out of CARE that may arise due to changes in circumstances or participation. These include:
- Transitions across counties.
- Dismissals due to incarceration, hospitalization, or non-participation.
Best Practices for Transitions
To ensure smooth and supportive transitions out of CARE, behavioral health teams are encouraged to follow these best practices:
- Begin planning early and proactively.
- Center the respondent’s voice and preferences.
- Ensure continuity of care and warm handoffs.
- Reinforce natural and community supports.
- Encourage creation of Psychiatric Advance Directives (PADs).
