Resources to Support CARE Act Implementation

CARE Act Statewide Reference Card

This statewide reference card is designed to support individuals working across a range of community-facing roles—including first responders, emergency department staff, behavioral health providers, and community-based organizations. It serves as both a quick-reference tool for these individuals and also a handout that can be shared with individuals and families during moments of crisis or concern.

The card includes two QR codes:

  • County Directory QR Code: Links to a statewide directory that helps users find local contacts and services based on their county. This ensures that individuals can quickly connect with relevant support, no matter where they are in the state.
  • Petitioner Resources QR Code: Provides access to information and tools for those considering filing a petition, including guidance on the process, available supports, and frequently asked questions.

This resource is intended to be used in the field—whether during a call, a hospital visit, a behavioral health interaction, or a community outreach event. It can be handed directly to individuals who may need help, as well as to family members or others who may be eligible to act as petitioners.

While this version is designed for statewide use, counties have the option to customize a template available in the Communications Toolkit to include county-specific information. This allows for local branding and the inclusion of county-specific resources while maintaining consistency in core messaging and access points.
